Button order form

1. Go to www.dspics.com.
2. Select your player’s picture number. It will start with DF06- ________
3. Fill in the information below and forward this to dave@dspics.com
4. Send me a check for $2.50 for each button, and I will send one check to David Silverman for the full order.
5. My address: Barb Dowdakin, 202 David Drive, Winnebago, IL 61088
6. I will have the buttons at the first home game. We may have a second order.
7. Deadline for the first order is this Friday, September 15^th!!!
8. Help on how to forward: Type in your information. Save it in Word as Button Order and your sons’ name (ex: button order for Jared), then attach that file to your email.
